PLEASE READ THE FOLLOWING INFORMATION CAREFULLY
The 1st SIGN-UP date will be: JULY 15, 2009 at the FARR WEST PARK BOWERY on the
Old Highway across the street from the old brown Church 4:00-5:00 p.m.
The following items will need to be FILLED OUT COMPLETELY, SIGNED (by members, parents and school officials) AND NOTORIZED WHERE NECESSARY AND RETURNED TOGETHER WITH YOUR APPLICATION:
1. NATIONAL MEMBERSHIP APPLICATION (FRONT AND BACK)
2. UHSRA MEMBERSHIP APPLICATION
3. SPIKERS MEMBERSHIP APPLICATION (FRONT AND BACK)
4. COPY OF YOUR CURRENT 4TH TERM REPORT CARD
5. MEMBERSHIP FEES AND ENTRY FEES FOR FIRST FOUR RODEOS.

Notary service will be available at signups for a $1.00 charge per entry form.
*Please note we sign up for more than one rodeo at a time so have all entry forms necessary for that week. You may write more than one check, because we signup for so many at one time.
If you cannot make it to the sign-up night please have your papers ready and send them with a friend or acquaintance.
LATE ENTRIES WILL NOT BE TAKEN!
**SPONSORSHIP THIS YEAR WILL BE A MANDATORY $175.00 FROM EACH MEMBER.
*SPONSORSHIPS ARE DUE NO LATER THAN JANUARY 06, 2010.
Sponsorships may be turned in at any sign-up on the proper completed form, which is on the website, or they may also be mailed to the secretary: Brenda Johnson 12306 N. 10000 W. Bothwell, Utah 84337. Please note that there are sizing guides on the sponsorship form. YOU will be responsible for turning IN AN AD THAT IS READY TO PRINT. If it does not fit into the sizing guide for the amount of sponsorship, it will be returned to you.
AUGUST 19, 2009 at 5:30.m. – GENERAL SPIKER MEMBERSHIP MEETING Following sign-ups at the FARR WEST PARK BOWERY. Agenda will include election of student officers, event directors, winter practices.
On the web are financial statements from the Rodeo and the Year-End Statement. We always work hard and look for as much help as we can in order to give more back to all our Spiker members. There are a lot of expenses involved and we want to do the best we can to help and benefit our club. Please look over the statements and if you have any suggestions or would like to see any changes, let us know.
*Please check the Spikers web site for updated information and posted weekly draws for your positions. Remember it is your responsibility to check your draws and make sure there are no mistakes. THE FALL RODEO SCHEDULE HAS NOT BEEN APPROVED BY THE EXECUTIVE BOARD, SO AS SOON AS IT IS I WILL POST IT! THE FIRST TWO RODEOS HAVE TO BE IN BY THE CLOSING DATE OF THE 24TH OF JULY, SO WE HAD TO HAVE REGISTRATION BEFORE NATIONALS!!!!
